Rat king by Brett Preston

A rat king is a collection of rats whose tails are intertwined and bound together by one of several possible mechanisms, such as entangling material like hair or sticky substances like sap or gum.

It's a metaphor for how twisted your brain gets during a crisis, brought on by mental illness, and perpetuated by drugs and alcohol.
The brain twists itself up with negative thoughts and feelings. Life is hopeless.
